There is some evidence that both American and Russian SSNs have fired in anger, but after that tit for tat exchange they agreed the official line is that both boats sank due to accidents.

The British Apaches were built by Westland. Not the Yanks. Everyone agrees the Brits have the craziest Apache door gunners - no doors so they strapped guys to the winglets.
Yanks did a lot of low level attacks in Vietnam and were quite capable of hitting the target, just not capable of pulling up in time! This may explain their teen marriage rate. :P
